We have a mobile response team that responds to crisis calls 24 hours a day and seven days a week. Individuals, families, schools, community agencies, and medical and emergency response personnel can call our Mobile Response Team for consultations, evaluations, and emergency support. Seek immediate help or call 911  if you or someone you know may be thinking about ending a life or hurting others. Making choices that put you or a loved one in danger, becoming unable to care for yourself, or struggling with addiction/substance use issues are other signs that may need immediate help. The Adams State University Food Pantry strives to decrease food insecurity on the Adams State University campus. The pantry is located in the Student Union Building in the CoRE Center next to the hair salon and mail room. This Food Pantry is FREE and open to all students, faculty, staff and the community. This includes online and graduate students.

Fresh fruits and vegetables can often be found in the refrigerator, and frozen fruits and vegetables found in the freezer so be on the lookout for those items.

Donations are accepted anytime. The pantry accepts food and personal hygiene products such as body wash, shampoo, conditioner, and toothpaste. Please call for donation drop offs.

Adelante supported families are offered individualized assistance in the navigation of resources that will help families set and reach their goals. Advantage Treatment Center’s Mission is to monitor and manage offenders in a residential and non-residential modality aligning them with treatment and vocational resources to help. CSFP is a free food program for people over age 60 who live in Alamosa County. Distribution is available once month.

The Food Bank Network of the SLV distributes CSFP (also known as Everyday Eats or Senior Commodities) and TEFAP programs for Alamosa County. TEFAP – The Emergency Food Assistance Program is a federal program that helps supplement the diets of people with low income by providing them with emergency food assistance at no cost.

Our programs include structured academic interventions offered during the school day and a more formalized after-school program, as well as increased staffing and a wrap-around support focused to reduce the students probability of engagement in high-risk activities. We strive to cultivate a culture of acceptance and accessibility in our organization, and branch out those practices to the communities we serve.

Amistad is an elderly and disabled day service facility dedicated to the physical and emotional needs of our clients. We provide a variety of activities such as crafts, bingo, physical exercise, and socialization. Our facility offers nutritional meals for our clients and we offer a variety of amenities to ensure our clients feel comfortable and supported. Our facility features comfortable seating areas, a dining hall for meals, and a variety of activity rooms for games, crafts, and socialization. We provide monthly transportation for a supervised visit to Walmart. During warm weather we provide a monthly day trip to get out and enjoy the beauty of the area we live.

Antonito Housing Authority provides housing to low-income individuals and families. We operate a socially and fiscally sound public housing agency that provides a drug-free, smoke-free, decent, safe and sanitary housing with a suitable living environment for residents and their families.

Since 1978, the SLV AHEC has been a leader of healthcare workforce development and community health programs throughout Colorado’s San Luis Valley. All six counties in the Valley are federally identified as medically underserved, and the AHEC’s goal is to strategically address this problem. The SLV AHEC is affiliated with the University of Colorado’s Anschutz Medical Campus AHEC Program Office. Some of the recent and ongoing programs of the SLV AHEC designed to improve health outcomes in the San Luis Valley include:
Workforce pipeline programming for youth to cultivate interest in the healthcare field and improve health and education outcomes; Professional development activities for health care providers and the community; Community health programming addressing the opioid crisis with health care providers, first responders and community to respond to the current opioid crisis; Naloxone overdose-reversal education and distribution; Participation in the state’s first rural comprehensive harm reduction program partnership; advocacy for those who are using opioids and their families; Community collaboration programs aiming to prevent mental, emotional, and behavioral problems; Other community health programs such as a community health worker cardiac risk assessment program and a home visitation program catered to the needs of the communities we serve. Ascension Counseling and also provides substance abuse treatment in English and Spanish. Programs and groups designed to treat DUI/DWI clients are available. We do not offer payment assistance. We accept cash or self-payment on a sliding fee scale (fee is based on income and other factors). Call us, we can help.

Avalon Dentistry provides general dental care for adults and children. The office provides exams and treatment for common problems with the teeth and gums.

This program helps pregnant people and their families quit smoking. Baby & Me Tobacco Free is an evidence-based smoking cessation program. Participants attend counseling sessions to quit smoking and stay quit. Earn diaper gift cards for up to 12 months. Other smokers in the baby’s home can also go through the program and earn gift cards. Must be 36 weeks gestation or less. Must be a current tobacco user, or quit since becoming pregnant or quit within 3 months of becoming pregnant.

We enable the people of our community to pursue lifelong learning through our responsive collections, electronic resources, and innovative services. Our welcoming and inclusive public libraries are cornerstones of our diverse communities where children and adults can experience personal enrichment and connect with one another. Our library offers a free membership. With your membership you borrow books and videotapes and borrow audio books on the Hoopla App.  We offer a reading hour for ages 3-6 on Tuesdays from 1-2 pm at the library. (Parents must be present at all times.) We are the Northern Saguache County Library District. Parents Must be present with all children under age 14. SLV Cooking Matters offers free weekly classes related to cooking healthy meals on a budget. Classes are in both English and Spanish, and can be virtual or in-person. Our audience is geared toward parents of young children, though adults and youth of all backgrounds are welcome and encouraged to attend. We collaborate with agency partners to offer nutrition classes to groups and families; please reach out if you would like to partner, as we are always seeking new connections. Have you wanted to learn how to grocery shop for your family on a budget? Do you want to learn basic nutrition and cooking tips? Classes are 45 minute sessions, and participants receive a $10 grocery gift card. Classes are in English and Spanish.

The Blanca/ Fort Garland Community Center hosts a food pantry for those who need it. This is a program of the La Puente Food Bank Network. Care and Share Commodities are distributed on the 1st Monday’s of the month at 10am. Part of La Puente Food Bank Network.

Blue Peaks supports people with intellectual and developmental disabilities in the San Luis Valley. Adult Programs provide 24-hour supervision, health care, recreational activities and programming that addresses individualized needs.

Early Intervention Services for children and families can include therapies, assistive technology devices, family training and counseling, health services and social work services.

Case Managers coordinate services and supports through a Service Plan. Case Management Agent Rocky Mountain Health Plans would be the first contact person. Service planning is done in a person-centered fashion and includes the person’s individual goals and desires.

Brave Spaces is a partnership program between SLVAHEC (Area Health Education) and SLV Pride. It is a program for youth ages 11-high school graduation to cultivate community and friendship in an environment where they get to be their authentic selves. With this program we hope to prevent suicide in the youth of our community.

We host free monthly events that participating teens request. The events take place across the valley, with a different location and themed event each month. The program encourages community-building, authenticity, self-expression, creativity and more.

U.S. Hwy 160 at Hwy. 17; and Moffat Town Park at 5th Street and Reynolds Avenue in Moffat; and Poncha Springs (Salida area) at Poncha Market, 6250 US HWY 285; and Salida Chaffee Shuttle at 54 Jones Ave. in Salida (about 1/4 mile East of 50 Burger on North side of the road). This “Alamosa to Pueblo” route also stops in Cotopaxi, Penrose, Pueblo West and the Pueblo Transit Center in downtown Pueblo. The “Durango to Grand Junction” route has stops in Bayfield, Durango, Ridgeway and Montrose. The Trinidad to Pueblo route has a stop in Walsenburg at 400 Main St. and Aguilar at 146 W. Main St. but there is no route listed from Alamosa to Fort Garland/ Blanca or Walsenburg. Purchase tickets by downloading the Bustang Outrider app to your phone or call 800-900-3011 for more information. You cannot purchase tickets on the website. To get to Denver, use the “Alamosa to Pueblo” route and then in Salida (at Chaffee Shuttle) switch busses to the “Crested Butte to Denver” route. Capulin Branch Library gives free library services to children, adults, and families in Capulin and Conejos County. The library also holds a weekly story hour for young children. The Carnegie Public Library, Monte Vista Branch is one of the few original Carnegie Libraries that still exist in the United States today. Many of the Carnegie Libraries that have not been demolished are now used for other means. This makes the Carnegie Public Library, Monte Vista Branch very special. It is also on the Colorado State Historical Register and the National Historical Register.

*Restorative Practices in Schools: We collaborate with San Luis Valley schools and after-school sites to help build cultures focused on Restorative Principles. Schools are all unique. As such, we design individualized timetables, trainings, and interventions for each setting to help resolve conflicts and address issues like bullying and truancy.
**Restorative Community Programs: Our collaborative intensive case management programs include Law Enforcement Assisted Diversion (LEAD) and other community programs designed to support individuals who are struggling with substance use that utilize harm reduction, wraparound, and restorative justice principles.

The Commodity Supplemental Food Program is available for the Elderly (60 and over and those who meet the financial guidelines). This program distributes food three times a month. We ask you to call 719-672-3332 to  instruct you on the procedure to receive your food box.

The La Puente homeless shelter has been one of the few organizations in southern Colorado to provide emergency shelter services. Since its founding in 1982, it has been an open door where people can step into safety. Homeless? Need a hot meal? Need supplies? There are services available for both 1) enrolled shelter guests and 2) the entire community!

1) Enrolled shelter guests have completed our intake process and background check. Guests are assigned a bed in a family room, a 1st floor accessible room, women’s dormitory, or men’s dormitory. They are welcome to a 30 day stay (extensions are provided on a case by case basis). Guests commit to our community living guidelines, including sober-living. Enrolled shelter guests are assigned a case manager to work on individualized goals, whether that be housing, employment or something else. During their stay, enrolled shelter guests have access to these resources: – Breakfast, lunch, and dinner – Supplies such as seasonal gear, hygiene items, and clothing – Laundry service – Mail service – 24/7 access to common rooms and restrooms – Storage for valuables and medications – Limited storage for other personal items

2) The following services are offered to the entire community! – Lunch & Dinner: Home cooked meals are offered from 12pm-1pm and 5pm-6pm, 365 days a year! To go meals are available to those who need it. – Special Holiday Meals: We host special meals at lunchtime during the holidays such as Thanksgiving and Christmas. – Supplies: You can pop by and ask staff for basic supplies. We usually have the following new or gently-used items in stock: clothing, shoes, underwear, seasonal gear, shampoo, conditioner, razors, toothbrushes, diapers (size 1-pullups), deodorant, soap, and blankets. – Showers: Need a hot shower? You can sign up for a shower slot. Just ask one of the Shelter staff! We can also offer you a towel, shampoo, conditioner, soap and other hygiene items. – Mail Service: Don’t have a permanent address? Public housing was established to provide decent and safe rental housing for eligible low-income families, the elderly, and persons with disabilities. Public housing comes in all sizes and types, from scattered single family houses to high-rise apartments for elderly families.
Public Housing is limited to low-income families and individuals. A Housing Authority determines your eligibility based on: 1) annual gross income; 2) whether you qualify as elderly, a person with a disability, or as a family; and 3) U.S. citizenship or eligible immigration status. If you are eligible, the HA will check your references to make sure you and your family will be good tenants. HA’s will deny admission to any applicant whose habits and practices may be expected to have a detrimental effect on other tenants or on the project’s environment. The HA representative will request whatever documentation is needed (e.g. birth certificate, tax returns) to verify the information given on your application. The Public Housing Authority will also rely on direct verification from your employer, etc. You will be asked to sign a form to authorize the release of pertinent information to the Public Housing Authority.

Crisis Prevention helps those on the verge of homelessness stay within their homes while also assisting shelter guests on their paths out of homelessness. We do this by providing assistance with rent, mortgage, utility, energy, medical and crisis expenses. We also help with employment readiness through our ReHire service. The Crisis Prevention team works in partnership with clients to identify and overcome hurdles so they can live self-sufficiently in an economically struggling community. We seek to give hope to families when they need it most.

If you’re unsure whether we can help, we encourage you to reach out—it’s always worth asking!

Our campus in Alamosa has the following programs: Withdrawal Management (Detox); DUI Education/Treatment; Opioid Treatment Program (MAT) Methadone/Suboxone/Sublocade. An outpatient program, Medication Assisted Treatment (MAT) can cut down on the amount of time you’ll need to invest in recovery. It also eliminates much of the withdrawal symptoms associated with early recovery, so patients may pursue treatment in relative comfort. La Puente’s Food Bank Network of the San Luis Valley provides nutritious emergency food to those in the San Luis Valley, through 15 food pantries spread across 8,000 square miles.

MedRide gives rides to people all across Colorado for medical appointments, school, and other important trips. We help people with Medicaid and private pay, that need safe transportation. We focus on making rides safe, on time, and comfortable for everyone. Medical transportation runs 24/7.

For local appointments, we recommend calling us at least 48 hours in advance. For any appointments 25 miles or beyond from the pick up point, the Health First Colorado Medicaid program has implemented a state required form to be filled out by your medical provider’s office.

For appointments more than 25 miles away from your pick up point, we recommend contacting us within at least 2-3 weeks of your appointment date with the name of the facility, facility address, their fax number, the name of your medical provider, and your appointment date and time, so we can send the form to them in a timely manner.

Nurse-Family Partnership (NFP) is a free program for low-income, first-time mothers. Nurse Home Visitors serves mothers and their children throughout all six counties of the San Luis Valley (plus Fremont and Custer counties). Services begin in pregnancy and proceed until the child’s second birthday. This is a nationwide program.

We provide home health services for children and young adults on Medicaid. Our services include Certified Nursing Assistants (CNAs) and Private Duty Nurses who care for people in their homes.

We also offer In-Home Support Services (IHSS) and Community First Choice services, such as help with daily care, personal care, and basic health needs.

We teach parents how to care for their child and can hire them to care for their loved one. We work closely with your case manager at Rocky Mountain Health Plans to help start services and get approval.

The Positive Activities Lead to Success (PALS) Children’s Program is a licensed after-school and summer program for children in kindergarten through fifth grade who have high social-emotional needs. We provide a nurturing and exploratory environment. The program is tailored to the needs and interests of each child. PALS is designed to empower children by strengthening their sense of self, connecting to family and engagement with their community. Through creative activities and enriching experiences, PALS encourages personal growth, self expression and resilience. PALS offers family support in the form of goal-setting,  educational opportunities for families, and collaboration with community organizations. PALS graduates can apply to support the program by mentoring, attending field trips and engaging in community service.

Children are accepted into the PALS program on a referral basis. These referrals can come from the Adelante program, the shelter, doctor’s offices, parole agents, counselors, the Alamosa County Department of Human Services and others. See referral link below. Most students come from the Alamosa area, but all students are welcome.

Parents as Teachers promotes the early development, learning and health of young children by supporting and engaging their parents and caregivers. Alamosa County Public Health nurses provide sexual education presentations in the Alamosa High School classroom setting as part of the Health Class curriculum. FLASH is a widely used sexual health education curriculum developed by Public Health. FLASH is available for high school classrooms. The Family Living and Sexual Health (FLASH) curriculum is designed to prevent teen pregnancy, STDs, and sexual violence, and to increase knowledge about the reproductive system and puberty. High School FLASH has been proven effective by rigorous evaluation.
FLASH is unique in many ways. It is a proven, evidence-based sexual health education curriculum designed to prevent pregnancy, STDs and sexual violence. It is deigned to be used in school classrooms, as a part of a health unit, although it can be successfully implemented in a variety of environments. It does not require training, and provides substantial teacher support so that it can be immediately implemented by any school that is ready. It includes a strong family involvement component, creating opportunities for families to talk with their children about important sexual health topics. It is an inclusive curriculum, including examples and activities that will resonate with youth from a variety of geographical regions, racial identities, and sexual orientations. It is highly interactive and is respectful of students with a variety of sexual experiences. The curriculum can be used in schools or for other groups.

The SLV Immigrant Resource Center offers the Por Ti Misma program assisting immigrant survivors /crime victims. We prepare all necessary paperwork for work authorization and legal residence, and then submit paperwork to USCIS. We provide referrals for support and counseling in domestic violence and sexual abuse cases. We assist crime victims to apply for VAWA (Viiolence Against Women Act) or the U-visa if eligible and help survivors of crime increase find and retain safe, long-term housing. Confidentiality is assured. Non-governmental and services provided free of charge.

The CCPP focuses on impacting community-level change to prevent substance abuse and works to change peoples perceptions, attitudes and habits around substance abuse.
Committee members participate in and provide direction for each step of the prevention process from planning through implementation. The CCPP committee meets monthly and welcomes interested participants at any time.
CCPP is optimistic about its capacity to bring positive change to Costilla County. The committee is an open group and parents, teachers, officials, youth and other community members are encouraged to join us and find out ways you can be involved in this process.

Project Healing Waters is a fly-fishing social/ recreational opportunity for veterans  group. In the winter they build rods, tie flies and provide lunch. Please join us.

The SLV Immigrant Resource Center offers the Punto de Vista program providing outreach and education opportunities to immigrants in the San Luis Valley. We are working toward better communication and awareness between the immigrant community and the broader SLV community. Early Head Start (EHS) programs serve infants and toddlers under the age of 3, and pregnant women. EHS programs provide child development and family support services to infants and toddlers and their families, and to pregnant women and their families. The San Luis Valley Behavioral Health Group (SLVBHG) offers many mental and behavioral health services across the San Luis Valley. Individual and group counseling is available, as well as Medicated Assisted Treatment for substance misuse disorders.

Although we provide various outpatient services, there is no inpatient Psychiatric Care in the San Luis Valley. Mi Esperanza Wellness Center is located at 311 San Juan Ave in Alamosa and is open Monday to Friday 8 – 5. This is drop-in “living room” available for self-help and peer support.

SLV Community Solutions is a nonprofit, community-based civic corporation operated to promote the self-sufficiency and the economic, social, political, and emotional well-being of the communities and the residents of the Region of the San Luis Valley. We collaborate with other organizations to build bridges, build capacity, and help fill gaps collectively with other organizations and institutions.

The San Luis Valley Communications Center (State Patrol Dispatch) uses the “Everbridge” system to issue notifications when there is an emergency situation in a particular area (previously “Reverse 911” and “CodeRed”). You MUST SIGN UP to receive notifications. During some emergencies, local authorities will share information to the public on the website and on social media. Some emergency messages, like Amber Alerts, use a different system that messages phones in a certain area. To receive those notices, make sure the settings on your phone are set to ALLOW emergency notifications. This does not work on all cell phones.

The Habitat for Humanity ReStore in Alamosa is a nonprofit home improvement store and donation center. Habitat for Humanity also helps people in need of decent, affordable housing. Adults can apply for homeownership with their local Habitat for Humanity although the process can take more than a year to complete. The local Habitat’s family selection committee selects homeowners based on the applicant’s level of need, their willingness to partner with Habitat and their ability to repay a mortgage through an affordable payment plan. Habitat’s homebuyers invest hundreds of hours of their own labor alongside volunteers and other Habitat homeowners. Neither race nor religion are factors in choosing Habitat’s homeowners.
Our Habitat for Humanity ReStore in Alamosa is a nonprofit home improvement store and donation center that sells to the public at a fraction of the retail price. Profits from the Restore in Alamosa fund our affordable housing program and sells new and gently used furniture, home accessories, building materials and appliances to the public. SLV Home Health is part of Alamosa County Public Health Department. We provide high-quality, compassionate home health care with skilled nursing, physical therapy, occupational therapy, and aide services.
A physician order is required to access these services.
Home Health is Medicare/Medicaid/ HealthOne certified and accepts most private insurances.
Registered Nurses and Licensed Practical Nurses provide individualized plans of care to fit the client’s needs which may include:

• Skilled Nursing: Including but not limited to skilled assessment, patient education, wound care, medication management, diabetic instruction and care, phlebotomy, IV therapy, PICC line care, and co-morbidity management.
• Physical Therapy: Including general strengthening, gait training, motion exercises, home exercise program development, post-surgical care, post-stroke and cardiac rehab.
• Occupational Therapy: Therapeutic exercises, activities and strategies to improve activities of daily living and optimize independence and safety; training and intervention to enhance memory, attention and problem-solving.
• Certified Nursing Aides: Skilled bathing and personal care services, simple therapy exercises under the instruction of an RN or PT, patient monitoring, and skilled transfer.

Housing assistance programs; Home Rehabilitation enables eligible applicants to make necessary repairs on their home to bring the home to Housing Quality Standards.
The Down Payment Assistance program is a homeownership program providing homebuyers with a low-interest loan for the down payment and closing costs for the purchase of their home.
Casita de la Luna Apartments – On-site management, a community building, a laundry facility, and beautiful landscaping are just a few of the amenities of the complex. These apartments are exclusively for the elderly and disabled.
High Valley Manor Apartments – A multi-family housing complex with 1 or 2-bedroom units. A community room, laundry facility, and beautiful landscaping are just a few of the amenities of the complex.
Century Mobile Home Park – A 185-space park with a mix of homeowners and renters. Lot rent, for homeowners, is available for those who need to bring a home into the park.

Regional Emergency Medical and Trauma Advisory Council (RETAC) supports quality care and consistent protocols across regional emergency services. Our mission is to develop, foster, and support excellence in trauma care and the emergency medical system within the San Luis Valley. The San Luis Valley RETAC is comprised of members representing ten EMS agencies, three hospitals and related partners in the San Luis Valley region of Colorado.

SMP’s primary focus is to teach older Americans on how to recognize and report Medicare fraud, errors and abuse. Medicare loses billions of dollars each year. That’s our Medicare! SMP assists with resolving billing issues, assistance with appeals, educate how to safeguard their personal information. When the need arises for issues to be reported, SMP reports to the state, who in turn reports to the OIG, so complaints and issues do not fall on deaf ears.

Supplemental Nutrition Assistance Program (SNAP) is the food assistance program in Colorado, formerly known as Food Stamps. SNAP provides food assistance benefits as part of a federal nutrition program to help low-income households purchase food.

South Central Colorado Seniors, Inc. is the Area Agency on Aging for Region 8. As an Area Agency on Aging, we respond to the needs of older adults and their family caregivers in the communities we serve. We coordinate low-cost comprehensive quality care by helping older (60+) and their families navigate the complex system of services. We serve people in Alamosa, Conejos, Costilla, Mineral, Rio Grande and Saguache Counties. Services include nutrition, caregiver support, material aid (eye glasses, walkers, etc.) SHIP, homemaker services, care management, information, assistance and referrals and a Long Term Care ombudsman (advocate for residents in long-term care facilities, assisted living centers and swing beds). Our Street Outreach works with community members living on the streets and other places that tend to be transitional within the San Luis Valley. We offer distribution days at St. Benedict’s Encampment and other locations around the Valley, during which we ensure unsheltered individuals receive clothing, food, water, camping gear, propane, and other items needed for tougher living situations. We also offer case management to help connect unsheltered individuals to various resources and provide an advocating voice.
