We serve both Rio Grande and Mineral counties. To apply for any of our programs you may come into our office and pick up an application or fill-out and print an application online:

• Pick-up an application at the Department of Human Services office.
• Or, Complete an application online. http://colorado.gov/PEAK
• Or have an application mailed to you by calling the office.

Apply for food assistance, Medicaid, long-term care, Children’s Health Plan (CHP), financial services — Colorado Works, State Aid to the Needy and Disabled and transition to Social Security Income; Old Age Pension (OAP). We may be able to help with Child Support. Se habla espanol. Some programs require citizenship. See our website for more information.
We are in Mineral County on the first Tuesday of every month from 1 pm to 3 pm in the Public Health office in Creede at 802 Rio Grande Ave. If you want to make an appointment to visit when staff is in Creede please call 719-658-3381.
