The OLTC program assists Medicaid-eligible people with functional needs to stay in their homes. Case managers assess functional needs and abilities and assist clients to access services such as personal care, homemaking, home modifications, adult day services, non-medical transportation and client-directed attendant support services, CDASS, home care allowance, and respite care. For Medicaid-eligible elderly, blind or disabled persons only. Our toll-free number is 800-346-4643. For an intake interview appointment write to CMAIntake@uhc.com
Persons that are elderly and/or have other functional needs are assessed for alternatives to Nursing Home Care. These services may include homemaking, personal care, home modifications, electronic monitoring, adult day services, non-medical transportation, alternative care facility, respite for caregivers and attendant support.
