There is an on-site pharmacy with delivery service available for most prescriptions to one of our clinics. Our primary care clinics are located in Del Norte, Monte Vista, South Fork, and Creede. Our Wellness Center is NOW OPEN. The Wellness Center is to advance health, wellness, and healing, moving beyond sick care treatment to prevention and well-being. The facility includes: A kitchen for healthy cooking and educational classes for healthy eating, Fitness room to try out different equipment, Private meditation rooms, Meeting rooms, a community lounge, a large community conference room, other patients, community, and staff-focused amenities. In 1996, Rio Grande Hospital at Del Norte re-opened at the old St. Joseph Hospital Building. The Rio Grande Hospital moved to its present location on August 11, 2004.
