Since 1999, SLV Transportation/Red Willows has provided 24/7/365 non-emergency medical transportation to our deserving clients. We can take you anywhere in the state. Services offered include medical and non-medical transportation. We accept private pay clients, and Medicaid or Worker's Compensation payment but you must call us or come in to check your eligibility. If you are eligible, then you need to be certified by your own primary care physician. The Primary Care Physician must be established in the San Luis Valley. Medicaid requires that the clients see the nearest Medicaid Provider to be eligible for transportation. This paperwork must be filled out by a Primary Care Physician and the client. (Paperwork consists of the Certification sheet, nearest provider form, consent form and HIPPA form). Human escorts and certified service animals may accompany the client if we are notified when you make your appointment for transportation.
