We provide home health services for children and young adults on Medicaid. Our services include Certified Nursing Assistants (CNAs) and Private Duty Nurses who care for people in their homes.
We also offer In-Home Support Services (IHSS) and Community First Choice services, such as help with daily care, personal care, and basic health needs.
We teach parents how to care for their child and can hire them to care for their loved one. We work closely with your case manager at Rocky Mountain Health Plans to help start services and get approval.
